At McCrimmon Primary Care Plus, CCM extends the reach of your primary care team — offering regular check-ins, care coordination, and personalized support between visits. The goal is to help you stay healthier, avoid complications, and feel confident managing your conditions.
CCM is available to Medicare and Medicare Advantage patients who have two or more chronic conditions such as diabetes, high blood pressure, heart disease, arthritis, asthma, COPD, depression, and more. Participation is voluntary, and our team will review eligibility, benefits, and any potential costs with you before enrollment.
To begin the process, patients should schedule an appointment with a provider to review medical history, confirm eligibility, and discuss whether CCM is right for their care needs.
