Eligibility for CCM is determined by a provider for Medicare and Medicare Advantage patients with two or more chronic conditions expected to last at least 12 months. These conditions may require ongoing management, coordination of care, and monitoring to reduce complications and hospitalizations.
Enrollment begins with an appointment with your provider. During this visit, your provider will review your medical history, determine eligibility, explain the program in detail, and obtain your consent to participate. From there, a care plan will be developed to support your ongoing health needs.
